Want to play sports next year? You must have a parent/guardian complete the sport selection in FinalForms!

Unless selected and completed in FinalForms, families cannot get coach email reminders for strength + conditioning, practices, gear/team stores, or be cleared to participate. It will not keep your choices from previous years. Adults and students have forms to complete. (This includes signing all mandatory forms such as concussion, health, and parent consent forms that activate when a sport is selected.)

How to update your sports for 2024-2025:

• Parent/Guardian must log into FinalForms – www.jesuiths-ca.finalforms.com on a computer, not a cellular device
• Choose the 2024-2025 school year, not the 2023-2024 school year (Blue button at the top of your homepage screen)
• Click “Update Forms”
• Select your sport option(s) for 2024-2025
• Review and sign the new athletic forms that now populate once you choose a sport for 2024-2025
• Students should also review and complete any new forms that populate after a sport is chosen.
